2. Send via email the URL of your business website:

3. Send via email Business Owner/s Full Name.
4. Send via email Business Contact Phone Number:

5. Send via email your Business Contact Email Address: Example:

US Mail
All information sent via US Mail shall be in handwritten or typed format and all image shall be in digital/electronic format and shall meet All the following guidelines contained below.
!!IMPORTANT NOTE: Pictures/Images on glossy paper or printer paper WILL NOT be accepted via US Mail!!
1. Send via US Mail a picture of your company's logo or a picture
to represent your business in standard picture format.

All images/photos shall be on cdrom or diskette.

6. All information, data, cdrom, disks, diskette shall be enclosed in a
self-stamped envelope, and shall include:
a. Senders Full Name and/or Business Name of Return Address.
